Pinheiro</b></font></p>     <p>&nbsp;</p>     <p><font size=5><b>A</b></font><font size="3"> polui&ccedil;&atilde;o ambiental    pelo merc&uacute;rio, resultante da utiliza&ccedil;&atilde;o desse metal na    atividade garimpeira de ouro, &eacute; um exemplo importante de amea&ccedil;a    &agrave; sa&uacute;de de popula&ccedil;&otilde;es da Amaz&ocirc;nia decorrente    da ocupa&ccedil;&atilde;o acelerada dessa regi&atilde;o pelo homem nas &uacute;ltimas    d&eacute;cadas. Os garimpeiros e outros profissionais ligados &agrave; chamada    &quot;queima do ouro&quot;, est&atilde;o expostos a n&iacute;veis extremamente    t&oacute;xicos de vapor de merc&uacute;rio. A exposi&ccedil;&atilde;o ao vapor    de merc&uacute;rio origina a &quot;doen&ccedil;a do chapeleiro maluco&quot;,    personagem da Inglaterra vitoriana descrita no famoso livro de Lewis Carrol,    Alice no pa&iacute;s das maravilhas. Na Inglaterra dessa &eacute;poca, os chap&eacute;us    eram tratados no vapor de merc&uacute;rio met&aacute;lico para atender aos requisitos    de eleg&acirc;ncia. Com anos de exposi&ccedil;&atilde;o ao vapor, os chapeleiros    desenvolviam uma forma cl&iacute;nica de mercurialismo caracterizada por exuberante    sintomatologia neurol&oacute;gica. A exposi&ccedil;&atilde;o cr&ocirc;nica ao    vapor de merc&uacute;rio met&aacute;lico compromete caracteristicamente o sistema    nervoso, inicialmente com sintomatologia inespec&iacute;fica e, posteriormente,    com dist&uacute;rbios caracter&iacute;sticos da motricidade - tremor de pequena    amplitude, paresias, disreflexia e dificuldade de coordena&ccedil;&atilde;o    motora (1).</font></p>     <p> <font size="3"> A doen&ccedil;a decorrente da exposi&ccedil;&atilde;o    ao vapor do merc&uacute;rio met&aacute;lico em garimpeiros da Amaz&ocirc;nia    tem sido documentada em diversos trabalhos recentes (2-7). Contudo, a preocupa&ccedil;&atilde;o    maior do ponto de vista epidemiol&oacute;gico &eacute; a exposi&ccedil;&atilde;o    das comunidades ribeirinhas a compostos organomercuriais, como o metilmerc&uacute;rio.    O metilmerc&uacute;rio surge a partir da biotransforma&ccedil;&atilde;o do merc&uacute;rio    met&aacute;lico despejado na natureza por ocasi&atilde;o da garimpagem do ouro    ou proveniente de outras fontes, sendo incorporado &agrave; cadeia alimentar,    notavelmente nos peixes carn&iacute;voros, fonte de alimenta&ccedil;&atilde;o    dos ribeirinhos. A express&atilde;o cl&iacute;nica da exposi&ccedil;&atilde;o    humana ao metilmerc&uacute;rio e outros organomercuriais &eacute; potencialmente    grave, com repercuss&otilde;es neurol&oacute;gicas muitas vezes irrevers&iacute;veis    (8-10). Essa patologia foi descrita pela primeira vez no Jap&atilde;o, onde    ocorreu a contamina&ccedil;&atilde;o da ba&iacute;a de Minamata pelos dejetos    de uma ind&uacute;stria qu&iacute;mica, seguindo-se a contamina&ccedil;&atilde;o    dos peixes consumidos em grande escala pela popula&ccedil;&atilde;o local e    ficou conhecida como s&iacute;ndrome de Hunter-Russell ou doen&ccedil;a de Minamata    (8,9,11,12). A exposi&ccedil;&atilde;o cr&ocirc;nica aos compostos org&acirc;nicos    do merc&uacute;rio acarreta grave comprometimento do sistema nervoso, com efeitos    diversos em adultos e crian&ccedil;as que foram expostas nas fases precoces    do desenvolvimento do sistema nervoso, tanto pr&eacute; como p&oacute;s-natal    (13). As les&otilde;es no sistema nervoso central do adulto s&atilde;o focalizadas    e atingem principalmente o cerebelo e o c&oacute;rtex visual. O sistema nervoso    perif&eacute;rico &eacute; comprometido, especialmente em intoxica&ccedil;&otilde;es    mais graves. A sintomatologia indica comprometimento motor, somest&eacute;sico,    auditivo e visual. </font></p>     <p><font size="3">O metilmerc&uacute;rio ocasiona um quadro muito mais grave em    crian&ccedil;as do que em adultos, sendo as les&otilde;es do sistema nervoso    central generalizadas, em contraste com o aspecto mais focalizado observado    na intoxica&ccedil;&atilde;o do adulto (13). Podem ocorrer abortos por les&otilde;es    disseminadas do sistema nervoso central ou as crian&ccedil;as podem nascer com    paralisia cerebral grav&iacute;ssima (13). Nessas crian&ccedil;as ocorre comprometimento    do desenvolvimento neural, com altera&ccedil;&atilde;o da estrutura cerebral    e diminui&ccedil;&atilde;o do tamanho da massa encef&aacute;lica. O quadro inclui    tipicamente microencefalia, hiperreflexia, retardamento mental e motor grave,    podendo ocorrer cegueira e surdez. Os estudos realizados em comunidades ribeirinhas    da Amaz&ocirc;nia t&ecirc;m revelado teores elevados de merc&uacute;rio na carne    de peixes consumida por essas popula&ccedil;&otilde;es (14), e &iacute;ndices    elevados de merc&uacute;rio total e metilmerc&uacute;rio em amostras de cabelo,    configurando a exposi&ccedil;&atilde;o permanente nessas comunidades (15,16).    Particularmente na regi&atilde;o do rio Tapaj&oacute;s, v&aacute;rias comunidades    de pescadores e ribeirinhos t&ecirc;m exposi&ccedil;&atilde;o confirmada ao    metilmerc&uacute;rio, apresentando &iacute;ndices acima do limite de seguran&ccedil;a    estabelecido pela Organiza&ccedil;&atilde;o Mundial de Sa&uacute;de (13), 10    µm / g de cabelo, sendo o pescado da regi&atilde;o a fonte dessa exposi&ccedil;&atilde;o-    <a href="#TAB01">tabela 1</a> (15,16). </font></p>     <p><a name="TAB01"></a></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="center"><img src="/img/revistas/cic/v56n1/a24tab01.gif"></p>     <p>&nbsp;</p>     <p><font size="3">Existem v&aacute;rios relatos na literatura sobre os efeitos    t&oacute;xicos da exposi&ccedil;&atilde;o mercurial nas fun&ccedil;&otilde;es    visuais do homem e outros primatas. Nos estados tardios da intoxica&ccedil;&atilde;o    mercurial existe comprometimento severo do campo visual perif&eacute;rico (17).    Al&eacute;m disso, existem relatos de disfun&ccedil;&atilde;o da vis&atilde;o    central, compreendendo perda da sensibilidade ao contraste acrom&aacute;tico    para modula&ccedil;&otilde;es espaciais e temporais (18-24) e perda da discrimina&ccedil;&atilde;o    de cores (25,26). Os grupos de pesquisa de Neurofisiologia e Doen&ccedil;as    Tropicais, ambos da Universidade Federal do Par&aacute; (UFPA) e de Psicologia    Experimental, da Universidade de S&atilde;o Paulo (USP), associaram-se em projeto    conjunto para avaliar desempenho visual de garimpeiros de ouro e outros habitantes    de comunidades ribeirinhas da Amaz&ocirc;nia, os quais est&atilde;o expostos    a altos n&iacute;veis de merc&uacute;rio met&aacute;lico e/ou compostos organomercuriais,    em tarefas espaciais crom&aacute;ticas e acrom&aacute;ticas. Em S&atilde;o Paulo,    estudos semelhantes est&atilde;o sendo feitos em outras popula&ccedil;&otilde;es,    incluindo dentistas e trabalhadores industriais. Esses grupos de pesquisa est&atilde;o    trabalhando em estreita associa&ccedil;&atilde;o, tendo como objetivo sugerir    protocolos espec&iacute;ficos e sens&iacute;veis que possam ser usados para    detectar os est&aacute;gios precoces de disfun&ccedil;&atilde;o visual devida    &agrave; intoxica&ccedil;&atilde;o mercurial (5-7, 27-31). Na <a href="/img/revistas/cic/v56n1/a24tab02.gif">Tabela    2</a> s&atilde;o apresentados os resultados obtidos num grupo de garimpeiros,    ribeirinhos do Tapaj&oacute;s e t&eacute;cnicos de laborat&oacute;rio expostos    a n&iacute;veis elevados de merc&uacute;rio e cujo sistema visual foi testado    com t&eacute;cnicas modernas de psicof&iacute;sica computadorizada (5-7). Resultados    semelhantes foram obtidos com testes manuais em ribeirinhos do Tapaj&oacute;s    (23-24), assim como em trabalhadores de S&atilde;o Paulo utilizando psicof&iacute;sica    e eletrofisiologia computadorizada (27-30). </font></p>     <p>&nbsp;</p>     <p><font size="3"><I><b>Luiz Carlos L.
